By registering , I voluntarily agree to assume all of the fopregoing risks and accept sole res[onsibility fora ny injury to myself (includingm but not limited to, personnal injury, disability, and death), illnwaa, sMges, loss claim, liability, or expense, of any kind, that I may experience or incur in connection with my attendence at team tryouts or practicies ('Claims'). On my behalf, and on behalf ofmy family, I hereby release, covenant ot to sue, discharge, and hold harmless Jersey Express Basketball, LLC, its employees, agents, and represetnatives, of and from the Claims, including all liabilites, claims, actions, damages, costs or exprenses of any kind arising our of or relating thereto. I understand and agree that this release inckudes any claims based on the actions, omissions, or negligence of the organization, its employees, agients, and representatives, whether a before, during, or after participation in any organization event or program.
